Forms Builder

Add Forms

Creates a new form for your web site that collects information from users and sends to the assigned recipients via email. Can also be cataloged in a database.
To access the Forms Builder select Manage Forms from the Modules tab.

You will see a box titled 'Create New Form" with blank fields.

Name: Name your form.

Page URL: You may choose a unique file name for your form (ex. contact.html or contact_sue.html) or you may leave blank and Forms Builder will gene reate the file name for you from the Name field above.

Active: YES=visible; NO=not visible

Sort: Numeric value that determines in what order your form is displayed.

Create Form: Click this button to generate a totally blank form with no fields added.

Create Std. Form: Click this button to generate a form with typical fields already in place. Field names and variables may be changed at any time. *This option is a great time saver*

Now you will see your new form listed above.

Editing Forms

Log in to the Forms Builder as you did above.

Click the edit icon for the page you wish to edit. The edit icon is located to the far right under the tools title (looks like a pencil).

Edit an existing field by clicking on the edit icon located to the right side of the page.

You should now see the forms interface that includes existing questionnaire data; click the 'edit' button to the left of the field you wish to change. Make changes and then click 'save'.

ADD a new form field by clicking the ADD FORM FIELD link at the top of the page. Enter appropriate data and click 'save'.

Click SAVE to save and continue.

Configuring Forms

Log in to the Forms Builder as you did above.

Click the configure icon for the page you wish to edit. The configure icon is located to the far right under the tools title between the edit and delete icons.

Name: Name your form.

URL: Also known as “file name”. You may input your own file name, or leave this field blank and the Cmation system will automatically name the file using the “name” field data.

Active: yes=visible; no=not visible

Sort: Assign order of list placement using a numeric value. You may leave blank.

Email Form Results: yes=track form results; no=do not track form results

HTML Email: yes=HTML formatting allowed; no=TEXT only

Copy Poster: yes=copy of email sent to person posting; no=no copy emailed to poster

Human Filter: yes=user will be prompted to enter text to verify they are human, not a bot (reduces the amount of spam mail received); no=no prompt will be given, form will be sent immediately.

Email Subject: Enter the subject line you wish recipients to see when they receive your email.

Recipient: Enter the email address you wish to receive messages from this form.

CC Recipients: Enter email addresses, separated by commas, you wish to receive a carbon copy of messages rcvd.

BCC Recipients: Enter email addresses, separated by commas, you with to receive a blind carbon copy.

Database Form Results: Yes: information from form will be cataloged in a database; no=no data will be collected

Request Newsletter Signup: yes=include option to sign up for mailing list; no=do not include option to sign up for mailing list.

Redirect Results: yes=  ; no=

Redirect URL: You may choose to redirect users to a new page upon completion of the form. Input file path to the desired PAGE URL.

Thank You Message: Include a thank you message to users.

Deleting Forms

You can delete Content in two different ways:

Delete Option 1

The first way is to select the delete icon to the far right of the FORM NAME you wish to delete when you are on the Forms Builder main page in the admin.

You will be prompted with a confirm deletion page. Just select yes or no and click the confirm button.

Delete Option 2

The second way is to click on the edit icon to the far right (the pencil) of the FORM NAME you wish to delete and open the Forms Builder page. Scroll down to the bottom of the page and click the delete button.
